Good day my Name is Ms. Wieam Delacruz,38 Yrs old currently residing in Dagupan Pangasinan. I had graduated in University of Pangasinan Bachelor of Science in Nursing and Colegio De Dagupan associate in Computer Science. I work as an Family Private Assistant for 12 yrs and I also work as a Parttime healthcare Representative for 4yrs.Health coaching has been a part of my job because im dealing with the health of my patients..I am assisting my patients during medical checkup in the hospital. I choose the appropriate doctors that could make effective diagnosis of their diseases .I also assist them in Diagnostic procedure which is very important in their Diagnosis. I correlate with the Doctors about their Treatment and guide them in medical intervention and followup checkups. I answer all of their Questions with regards of their health. I see to it that the patient has been successfully treated and understand all about their health status. From time to time i help the patient until recovery stage. I handle many patient with different health cases. I am knowledgeable with their Health status as it was also endorse by their medical doctors. Being a effective Health coach is very important to help Assist the patient and help them in their Healthcare needs .I also have Good communication skills which is very important in gathering information in order to achieve effective treatment.being Compassionate is also important in my job by being Kind to my patient in rendering Care and Support to their health. Critical thinking is a must which means you are Knowledgeble enough in rendering appropriate care upon Diagnosis and Treatment of your patient.in order to achieve Optimal level of Health Effective Health Coaching is very Important...

Being a Family nurse i am always assisting tlmy patient in terms of their healthcare need.Im always making appointments with the respective Medical practitioner in the hospital.Im always accompanying them to visit the appropriate doctor on which they can ask everything about their health diagnosis.I will correlate with the Laboratory department in asissting their laboratories and other Medical Procedure that was underdone in the Hospital .The Doctor will then advise me on how to interrogate with their treatments and followups.I am assign to their treatment plan for their Proper Medication and health assessment..I will help them to improve their health condition and to answer all the inquiries with regards of their health and wellness.

I have a lots of experiences in my area of Nursing theres one time i have a patient whom she just want to have a checkup in the hospital.I choose her doctor bec i knew there was something wrong with my patient and they find out its assymptomatic nasopharyngeal cancer. She and her family were shock upon the diagnosis of the doctors which was also supported by clinical laboratories..they were thankful bec i guide them what kind of doctors are they going to visit.Upon diagnosis i was there assisting her during frequent checkups and chemotherapy and radiology treatment.She was recovered from cancer but then closely monitored . I also have a pediatric patient .this boy was complaining severe right abdominal pain for 2days and then the pain was gone..I instructed the family to visit a family medicine department to undergo whole abdominal ultrasound.It was confirm Appendicitis and need emergency surgery(Appendectomy).I assist him until recovery period.His family was happy and satisfied to my service Theres a patient of mine who complain of continous vomiting after alcohol intake.His blood pressure went high and the oral medication seems to useless .He vomit greenish substance .I instructed the family that he need to be confinement so that he is monitored frequently.His condition is called Gastro esophageal reflux syndrome and his bp couldnt went down so he need IV fluids and the medication must be administered fastly through IV Fluids .He was monitored and recovered and sent home
